New Patient Forms
You can handle most paperwork ahead of time — digitally, from home. It takes a few minutes and means less time at a clipboard when you arrive.
We use digital records, so the intake process is straightforward and can be completed before you ever walk through the door. Most patients find it takes just a few minutes.
You have two options: complete the forms at home before your appointment, or fill them out when you arrive in our reception area. Either way works.
If anything on the forms is unclear or you have questions while filling them out, just call us. We're happy to walk you through it.
What to bring
A short checklist for your first visit.
- Insurance card, if you have coverage
- Photo ID
- A list of any current medications you're taking
- Any relevant medical history you'd like us to know about
